You may request a University Lake School view book and application materials by following any of the steps below:
- E-mail our Admissions Department: admissions@universitylake.org
- Call our Admissions Department at 262-367-6011 - ext. 1455 or 1462
- Request an application in person, no appointment necessary.
- Download our Admissions Forms by using the links in the box above.
Application Process:
- Application: Complete and submit the application materials with a $25 application fee.
- Transcript Request: Submit the Transcript Request form to the applicant's current school.
- Writing Sample: 5th - 12th grade applicants must submit a writing sample.
- Teacher Recommendation: 1st - 5th grade applicants must submit a recommendation form completed by their primary teacher. 6th - 12th grade applicants must submit recommendation forms from two of their teachers.
- School Visit: Schedule a school tour, shadow day visit and placement test by calling 262.367.6011 ext. 1455 or 1462. Applicants are encouraged to shadow a current ULS student for the day.
